I had five responses to my survey. The feedback I received was the following:

  1. It looks good, format is easy to follow. Tip based on all the surveys I have done at school over the past 3 years (and it has been A LOT!) don’t make it too long or people will just not do it! I found anything more than 5 min and I left them!
  2. The survey was good length and to the point.  One question maybe should have (craft) after activity.  Well written and easy to follow instructions.
  3. Looks good. I would make the text box in the first qualitative question larger.
  4. I found #5 a bit confusing. Maybe because I don’t know the program though.
  5. Maybe add an additional age category as these days there are older moms as well as grandparents who are raising their grandkids as their kids.

In reviewing my survey I also found one spelling error where I had written “to” instead of “do”.

As a result of the feedback I received I made the following changes.

  1. Age groups were changed to <18, 18-24, 25-31, 32-38, 39-45, and 45+
  2. Fixed my spelling mistake
  3. I added a preamble to each of questions 3, 4, 5, and 6 to add a bit of explanation
  4. I changed the text box on question 7 to the larger text box
  5. I changed the wording to question 4 to make it easier to read

I have been working on a short survey related to the program I am hypothetically evaluating. I need feedback on the structure/wording/content of my survey.
